The Distinguished Service Award’s purpose is to recognize our incredible membership. Many of you may volunteer at your school, church or elsewhere in your community. We want to acknowledge the outstanding philanthropy of a middle school chapter, high school chapter, and the individual who volunteers the most throughout the year at the State Conference. By taking less than a minute out of your day and submitting this form, you will have the opportunity to win the Distinguished Service Award!
We will only be accepting service forms until March 23rd, 2012 so be sure to submit yours before then!
